A DAY IN THE LIFE

What does it looks like to work at DuMolin Community Living?

 

Walk in through the front door and be welcomed home!

 

Our residents come and go throughout the day and weekend to attend Day Programs, Work Programs and visit family and friends; some are more independent than others in their outings and we support them whenever we can so they can have free access to their community and community events. We also encourage fully staffed whole house outings to places like the beach, Sonoma County Fair, baseball games, dances, concerts and the like. If our staff is having fun then so are our residents! 

 

Residents that live in our homes have a variety of intellectual and developmental disabilities as well as physical impairments. Our homes are licensed through California Department of Public Health and are classified as ICF/DD-N (Intermediate Care Facility/Developmentally Disabled - Nursing) and ICF/DD-H (Intermediate Care Facility/Developmentally Disabled - Habilitative) facilities. We currently have full-time and part-time positions available at both types of homes. Full-time is considered to be 30 hours a week or more and comes with excellent Kaiser benefits and a generous sick and vacation package. 

DIRECT SUPPORT STAFF

Part-Time and Full-Time Available

DUTIES AND TASKS

  • Participate in client habilitation and Active Treatment

  • Administer medications and treatments as directed by the RN

  • Assist clients with ADLs as prescribed in the Individual Program Plans (IPPs) 

  • Assist clients in preparing for their daily programs

  • Implement assigned recreational activities 

  • Prepare client's meals and snacks according to regulatory guidelines, dietitian recommendations and physicians' orders 

  • Provide direct resident care: basic hygiene, personal care needs to all clients, so that clients reflect clean, odor-free personal self, with color-coordinated and age appropriate dress

  • Assist taking clients on field trips and other activities that get the clients out of the house and into the community

QIDP / ADMINISTRATOR

Full-Time 40 hours a week

DUTIES AND TASKS

  • Maintaining and updating monthly notes, quartlies, ISPs, change of conditions.

  • Auditing client charts

  • Ensuring face sheet information is current

  • Following up on individual service plan recommendations

  • Maintaining an open line of communication between interdisciplinary team members.

  • Monitor and update behavioral and restrictive consents

  • Updating client training plans and data sheets

  • Management and development of facility staff

  • Oversight of regulatory compliance within the houses.

  • Training, partnership, delegation, and communication with their House Managers

  • Participation in the monthly House Meetings and periodic company-wide in-service meetings.

  • Participation in the monthly House Meetings, and periodic company-wide in-service meetings.

  • Participation in the Human Rights commitee for client welfare

CERTIFIED NURSING ASSISTANT / HOUSE MANAGER

Part-Time and Full-Time Available

Paid comprehensive initial and on-going training provided

DUTIES AND TASKS

  • Participate and at times lead client habilitation and Active Treatment

  • Administer medications and treatments as directed by the RN - including feeding and medications via G-tube and J-tube 

  • Assist in taking clients to appointments and tracking medical needs and reporting them appropriately 

  • Assist clients with ADLs as prescribed in the Individual Program Plans (IPPs) & provide direct care

  • Assist clients in preparing for their daily programs

  • Implement assigned recreational activities 

  • Prepare client's meals and snacks according to regulatory guidelines, dietitian recommendations and physicians' orders 

  • Assist taking clients on field trips and other activities that get the clients out of the house and into the community

RN

Part-Time 15-20 hours a week

DUTIES AND TASKS

  • Develop, implements, sustains and coordinates healthcare services. 

  • Participates as an active member of the interdisciplinary team (IDT). 

  • Plans and implements nursing and heal-related education programs for direct care staff 

  • Acts as a clinical resource and staff support to the House Managers, QIDPs, IDT and direct care staff. 

  • Understands regulatory channels for ICFs and is a source of providing client care within the confines of said regulations. 

  • Performs as a nursing consultant to the ICF and follows up on all medical recommendations, documenting, and ISP meetings. 

  • Will be responsible for the oversight of two homes with a total of 12 clients. 

  • Participation in the monthly House Meetings and periodic company-wide in-service meetings.
